To help us provide the best care possible when you are on campus, it's important for us to have up-to-date medical information about you.

For Incoming Students

We must have your Immunization Form, and Tuberculosis Screening Questionnaire before you can register for classes.  to complete these forms.

The Illinois Department of Public Health requires that all students who enroll at a post-secondary educational institution submit proof of required immunizations (Diphtheria, Tetanus, Pertussis; Measles, Mumps, Rubella; Meningococcal), and that any student who does not provide such proof be precluded from enrolling for subsequent terms. (Immunization Code, Section 694.100)

• COVID-19 Vaccination: 51³Ô¹ÏÍø strongly encourages all students, faculty and staff to be fully up to date with their COVID-19 vaccinations and annual flu shots, including booster doses when eligible

For Current Students

Please resubmit your Immunization Form (PDF) if there have been updates since it was last submitted to the College. You can mail hard copies to Health Services, 51³Ô¹ÏÍø, Box K-88, 2 East South Street, Galesburg, IL 61401; or drop them by the Health Services clinic located at Furrow Hall, 175 West 51³Ô¹ÏÍø Street.

Athletic Physicals

Athletes at 51³Ô¹ÏÍø will not be allowed to participate in practice, competitions, or organized off-season training sessions without a yearly physical examination. and learn about insurance and other forms required to be a Prairie Fire athlete.

51³Ô¹ÏÍø and the Sports Medicine/Athletics Training program also provides many resources to suffered by varsity athletes.
